1) Who we are and how these terms work
These Terms of Service (“Terms”) govern your access to and use of the Palm View Health & Wellness website (the “Site”). “Palm View,” “we,” “us,” and “our” refer to Palm View Health & Wellness located at 1616 N Litchfield Rd, Suite A220, Goodyear, AZ 85395. By using the Site, you agree to these Terms and to our Privacy Policy (linked in the Site footer). If you do not agree, do not use the Site.
2) Not medical advice & no emergency use
- Informational only. Content on the Site is for general information about our programs and services; it is not medical or professional advice and is not a substitute for diagnosis, treatment, or individualized clinical judgment.
- No patient-provider relationship via Site. Viewing the Site, sending us a message through it, or receiving a response does not create a patient-provider relationship.
- Emergencies. Do not use the Site for emergencies. If you are experiencing a medical or mental-health emergency, call 911 or go to the nearest emergency department immediately.
- Outcomes. Recovery experiences vary. We do not guarantee specific results or outcomes.
3) Eligibility; using the Site
You may use the Site only if you are at least 13 years old (or the age of digital consent in your jurisdiction) and can form a binding contract. You agree to comply with all applicable laws while using the Site.
4) Communications & consent
By contacting us (e.g., email, phone, web forms), you consent to receive replies from us at the contact details you provide. Message/data rates may apply. You may opt out of non-essential communications at any time by using the provided unsubscribe method or contacting us.
5) Privacy
Our Privacy Policy explains what we collect and how we use it. It is incorporated into these Terms by reference. Please review it before using the Site.
6) Health information & sensitive data
Unless a page specifically states otherwise, do not submit Protected Health Information (PHI), Social Security numbers, payment card numbers, or other highly sensitive data through general Site forms or email.
If you become a patient, PHI will be handled through appropriate clinical systems and processes as explained to you at intake.
7) Intellectual property
The Site (including text, graphics, logos, images, and layout) is owned by Palm View or our licensors and is protected by intellectual-property laws.
Except for your personal, non-commercial use, you may not copy, modify, distribute, publish, or create derivative works without our prior written consent. All trademarks are the property of their respective owners.
8) Acceptable use
You agree not to:
- interfere with or disrupt the Site or the servers/networks connected to it;
- attempt to probe, scan, or test the vulnerability of the Site or circumvent security;
- use the Site to transmit malware, spam, harassing, defamatory, or unlawful content;
- scrape, harvest, or collect information from the Site except as allowed by robots.txt or with our written permission.

12) Limitation of liability
To the fullest extent permitted by law, Palm View and its owners, directors, officers, employees, and agents will not be liable for any indirect, incidental, consequential, special, exemplary, or punitive damages (including loss of data, profits, goodwill, or other intangible losses) arising out of or relating to your use of the Site.
13) Indemnification
You agree to defend, indemnify, and hold harmless Palm View and our affiliates, owners, officers, employees, and agents from and against any claims, liabilities, damages, judgments, awards, losses, costs, expenses, or fees (including reasonable attorneys’ fees) arising out of or relating to your use of the Site or your violation of these Terms.
14) DMCA / copyright complaints
If you believe content on the Site infringes your copyright, please email help@palmviewhw.com
with: (a) your contact information, (b) identification of the copyrighted work, (c) identification of the allegedly infringing material and its location on the Site, (d) a statement that you have a good-faith belief the use is not authorized, (e) a statement under penalty of perjury that your notice is accurate and you are authorized to act, and (f) your physical or electronic signature. We will respond consistent with applicable law.
15) Governing law; venue
These Terms are governed by the laws of the State of Arizona, without regard to its conflict-of-law principles. You agree to the exclusive jurisdiction and venue of the state and federal courts located in Maricopa County, Arizona for all disputes not subject to arbitration (if any).
16) Severability; waiver
If any provision of these Terms is held invalid or unenforceable, the remaining provisions remain in effect. Our failure to enforce any right or provision is not a waiver of that right or provision.
17) Entire agreement
These Terms, together with the Privacy Policy, are the entire agreement between you and Palm View regarding your use of the Site and supersede prior or contemporaneous agreements on that subject.
